Sharp employee eligible for referral bonus. LocationThis position is located in Serra Mesa. Hours8 hour shift: 8:00 am - 4:30 pm, including weekend rotation. Qualifications * Bachelor of Science in Nursing or in health related field preferred. * Current California Registered Nurse licensure required. * Current CPR certification required. * Certified Case Manager preferred. * Three-five years acute care or clinical experience in area of specialty required. * One or more years of Discharge Planning, Utilization and/or Case Management experience, preferably in an acute care/ Managed Care setting. * Proficiency with information systems and computer programs such as Microsoft office, etc. * SummaryDynamic and fast paced Case Management, DCP, UR department, that consists of professional nurses, social workers, and unlicsenced support staff. Potential candidate is competent to proficient in Nursing skills to: Perform data assessment, and comprehensive assessment of the patients condition and doctors plan, and excute safe and appropriate, individualized care coordination; is able to demonstrate critical thinking and decision-making throughout the nursing process including but not limited to the client?s health behaviors, cultural influences, health beliefs and values; is able to positively influences client outcomes by identifying problems requiring intervention, anticipating barriers, and tracking trends that produces negative influences; develops, documents problem focused care coordination, as well as implements a measurable, individualized, cost-effective, and realistic plan of care, in collaboration with client/family and members of the health care team; performance is consistent with defined Case Management performance indicators of quality of care, qualifications, collaboration, legal, ethical, advocacy and resource management; is skilled in client and individual conflict resolution; is competent in therapeutic communication for provision of client services, and team building. Physical RequirementsAbility to sit and stand, perform computer work minimum of 50 percent of the time, and unlimited bilateral manual dexterity. Good vision and hearing, and ability to speak clearly. Ability to lift up to 25 pounds. Prolonged use of phone and frequent phone conversations. |
